Admission Criteria
Who can live at our residential personal-care home?
- Any seniors with chronic health conditions who needs a higher level of care to monitor and manage their health and medications/treatments
- Any seniors who can no longer safely live independently in their current living situation
- Any seniors who are not thriving and happy in their current living situation
- Any seniors who have memory issues and are unable to care for themselves (nutrition, hydration, bathing, toileting, dressing, walking, medication use)
- Any seniors who have difficulty with cooking, laundry, bathing/grooming, toileting, and assistance with transfer/ambulation (activities-of-daily living (ADLs))
- Any seniors whose family members are stressed and overwhelmed and can no longer care for their loved one
- Any seniors whose family members live far way and are unable to care for their loved one
- Any seniors who do not want to live alone but want to be in a home-like setting and not a traditional institution or facility
- Any seniors who currently reside in an assisted living facility who are not receiving appropriate personal attention and health care services needed to function and thrive
- Any seniors who currently reside in a nursing home facility who are not receiving appropriate personal attention and health care services needed to function and thrive
- Any seniors who have suffered a fall and cannot return home due to safety considerations
- Any seniors who are currently in rehab (skilled unit) after a hospitalization and cannot return home due to safety and health considerations
- Any seniors who want to have a better quality of life
Tran's Senior Oasis specific admission criteria:
- Seniors must be ambulatory or ambulatory with assistive devices (wheelchair, cane, walker, or other prosthetic devices) or require a 2 person assist for transfers. Individuals must not be bedridden on the day of admission
- Seniors must not have a history of wandering or repeated attempts to leave the building
- Seniors must possess reasonable judgment to treat others with respect and kindness or must be easily redirected
- Seniors must not have a history of physical, verbal, emotional, inappropriate or abusive behaviors towards self and others
- Seniors must not have a history of disruptive (screaming, yelling), violent, or destructive behaviors towards self and others
- Seniors must not have a history of biting, hitting, spitting, pinching, scratching, or kicking caregivers or others
- Seniors must not be an active alcohol consumer or substance abuser
- Seniors must not be an active smoker
- Seniors may be a Respite placement (temporary)
